Looking for your vaccination record? If you were vaccinated in Connecticut you can now access your immunization record (or your children’s) through the CT Wiz self-service portal. Click on the green bar to enter your identifying information and then click submit. When you get to the next page click on the blue bar to get a verification code emailed or texted to you.
The information you enter must match the information in the state database in order to continue. If your record cannot be found call the DPH Immunization Program at 860.509.7929 for assistance.
Please note that your CDC card provided at the time of vaccination is considered an official record.
